If you have a negative sentence, the word 'inte' (not) usually comes after the frequency adverb, or the adverb itself acts as the negative.
Affirmative: Jag äter alltid lunch.
Negative: Jag äter aldrig lunch.
In questions, the verb moves to the front, but the adverb stays close to the verb: 'Äter du ofta lunch?'
Remember, Swedish is a V2 language. The verb must be the second element. If you start the sentence with the frequency adverb, the verb must immediately follow it: 'Ofta äter jag lunch.' This is a great way to add emphasis!
