In Egypt, you might hear عايز طبي (I want a medical [note]). In the Levant, people might say بدي مغادرة for a short leave. You can also say أنا مريض (I am sick) first. If you are very tired, say تعبان (tired/sick). But إجازة مرضية remains the most professional and universal term.
