From Old French 'hors mis', literally 'placed outside'. 'Hors' (outside) comes from Latin 'foris' (out of doors, abroad). 'Mis' is the past participle of 'mettre' (to put, to place), which comes from Latin 'mittere' (to send, to throw). The combination thus indicates something that has been 'sent out' or 'placed outside' of a general consideration or group. The usage as a preposition developed over time to signify exclusion.
